Dr Griffiths & Partners takes great care to safeguard personal data provided by its patients and processes such data fairly and lawfully in accordance with the Data Protection Act 1998.

We do not capture and store any personal information about individuals who access this web site, except where you voluntarily choose to give us your personal details via email or an on-line form.

In these latter cases, the personal information you give us is used exclusively by us for providing you with current and future information about our services.

We do not pass any of your personal information to outside organisations and/or individuals, except with your express consent.

We will take reasonable technical and organisational precautions to prevent the loss, misuse or alteration of your personal information.

We will store all the personal information you provide on our secure (password and firewall protected) servers. All electronic transactions you make to or receive from us will be encrypted using SSL technology (excluding our Comments & Suggestions and PPG Application to join forms).

Of course, data transmission over the internet is inherently insecure, and we cannot guarantee the security of data sent over the internet.

(9) Online Applications:

This refers to the following Online Applications:

Health Questionnaire, Change of Personal Details, Blood Pressure Monitor, Travel Risk Assessments, Smoking Status and Contact by Email.

The Internet is not a secure medium. While your questionnaire will be processed in strict confidence within our practice, we cannot guarantee its security over the Internet. However, we have taken a number of steps to help ensure the data you send to us is secure:

  1. We only request your Patient Number or part of your firstname, part of your surname and your date of birth to identify you. **
  2. We use an SSL (Secure Sockets Layer) link so that the data you enter is encrypted when sent over the communications link.
  3. As soon as we receive your data we delete it from our online server, so your data only remains on the Internet for a short period of time.

These measures ensure the applications are more secure than sending 'free text' emails. However, we do not guarantee or warrant these applications in any way. You are in no obligation to use the applications. If you do not wish to use these applications, please use one of our alternatives (eg paper copies of forms & questionnaires, telephone, fax, post or in person visits to the surgery).

(10) Comments & Suggestions Form:

This form sends an email message over an SSL (Secure Sockets Layer) encrypted link, it should however not be used for any confidential purpose. It is more secure than sending 'free text' emails and has a similar security level to sending an email message from your email application (e.g. Microsoft Outlook) over a secured link. However, we do not guarantee or warrant this form. We have provided a box in reception for paper based Comments & Suggestions. This is provided as an alternative to using the online version.

(11) Patient Participation Group Joining & Contact Form:

These forms send an email message over an SSL (Secure Sockets Layer) encrypted link. It is more secure than sending 'free text' emails and has a similar security level to sending an email message from your email application (e.g. Microsoft Outlook) over a secured link. However, we do not guarantee or warrant these forms. If you do not wish to use these forms please apply to join the group in writing to our Practice Manager.

(12) Policy Amendments:

We may update this privacy policy from time-to-time by posting a new version on our website.  You should check this page occasionally to ensure you are happy with any changes.

(13) Your Rights:

You may instruct us to provide you with any personal information we hold about you. Provision of such information will be subject to:

(a) the payment of a fee (currently fixed at £10.00); and

(b) the supply of appropriate evidence of your identity (for this purpose, we will usually accept a photocopy of your passport certified by a solicitor or bank plus an original copy of a utility bill showing your current address).

We may withhold such personal information to the extent permitted by law.

You may instruct us not to process your personal information for marketing purposes by email at any time. In practice, you will usually either expressly agree in advance to our use of your personal information for marketing purposes, or we will provide you with an opportunity to opt-out of the use of your personal information for marketing purposes.
