How to Register as a Patient
Registration Information
Your registration must be completed before you can book an appointment with us. Please allow up to five working days after submitting your form for the registration process to be finalized. We will send you a text message to confirm once you are registered with us.
Until your registration is complete, you will remain registered with your previous GP. Please note that we are unable to issue repeat prescriptions until we have received your medical history and medication details, so make sure to request a supply from your previous GP.
If you are registering children under 16 years old in your care, please contact the practice before completing the online registration.
Step 1: Complete a Patient Registration Form
Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.
Step 2: Register for Online GP Services and Upload Identification
Please register for Access to Online GP Services. To register for online services we do require a copy of your photo identification (passport, driving license or European Union Identity card). You can upload a copy of your identity with the 'Upload Identity' button below, or if you prefer to submit your documents in person please speak to the reception team.
Other Ways to Register
If you do not wish to Register Online you can download and complete the following:
- A completed Register with a GP Surgery (PRF1) form.
- This should be completed with your NHS Number (we are unable to register you without your NHS Number, you can get this from your previous GP Surgery).
- Proof of Identification (ID) (Photographic ID, for example Passport or Driving Licence)
- Proof of Address (recent Utility Bill, i.e. gas, electric or council tax bill)
When complete please take your information to the practice reception. Copies of our registration form are also available from reception.
Registering your New Baby
Please register your new baby at the practice as soon as possible. Download and fill out the Register with a GP Surgery (PRF1) form, or pick up a copy from reception. Return the completed form to reception and include your baby’s NHS Number, which can be found in their Red Book.
Once registered, we’ll send you an appointment for a mother and baby post natal check. This will take 50 minutes; 30 minutes for your baby’s check and 20 minutes for yours. Your baby’s immunisations will start at 8 weeks and will be done at this appointment.
Don’t forget to bring the Red Book. If you can’t make the appointment, let reception know as soon as possible. Please note, rescheduling may mean waiting up to 4 weeks for the next available slot.
Temporary Residents
The Doctors are happy to see patients who are temporarily resident in the Practice area should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.
To register you will need to bring your passport and complete a Temporary Resident Registration form which is also available from reception.
How to Find Your NHS Number
An NHS number is a 10 digit number, like 485 777 3456.
Your NHS number is unique to you. It helps healthcare staff and service providers identify you correctly and match your details to your health records.

New Registrations Check
All newly registered patients are advised to attend for a health check with the Practice Nurse or our Phlebotomist, this will provide details of your medical history from the start. Ask Reception for details.
Valid forms of Identification
To register we require photo identification (ID) (passport, drivers licence or European Union Identity card).
If you are homeless or of no fixed abode, then although it is helpful to have photo ID you can register without any.
Any other ID such as an HC2 Certificate, Rough Sleepers' Identity badge, Hostel registration or mail forwarding letter would also be helpful but again we will register you even if you don't have these. As we know it is more important that you can access health care when you need it.

Accountable Named GP for All Patients
All patients registered at Cumberland House Surgery have a named doctor who has overall responsibility for your care and support. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.
