Sickness certificates (also known as “Fit-notes” and Med 3 Forms) can be issued at routine GP appointments. For the first seven days of any illness or incapacity a self-certification form (SC2) should be used and are available from your employer or our reception. Hospital doctors can issue Med 3 forms following out-patient appointments or hospital stays.
